A couple of things.</p> <p>The theme appears to contain an error. When clicking on "pointsixty options" from the Appearance menu the error is produced. It is traced to this line in functions.php ...</p> <p><code>$theme_data = get_theme_data(ABSPATH . 'wp-content/themes/kozuka/style.css');</code></p> <p>Also, you should put the theme files in a theme folder.
